What is the List of Surviving Family Members used for?

It identifies the surviving family members of a deceased employee or pensioner so the AG Office can determine who is entitled to the family pension and in what share. It is a key document in every family-pension case.

Who needs to fill out this form?

The legal heirs of a KP government employee or pensioner who has died, when claiming the family pension. It is filed by:

• The widow / widower of the deceased
• Children of the deceased employee
• Dependent parents, where applicable
• The nominee or guardian on behalf of minor heirs

What information does the form ask for?

The form lists each surviving family member with the details the accounts office needs to assess entitlement:

• Name of each family member and relationship to the deceased
• CNIC of each adult member and date of birth of minors
• Marital status of children and dependent status
• Date of death of the employee / pensioner
• Attestation by a gazetted officer or authorised official

What documents are required?

Attach the supporting documents so the AG Office can verify the family and process the claim:

• Death certificate of the employee / pensioner
• CNIC copies of all adult family members
• Form B / registration certificates of minor children
