What is the GP Fund Number Allotment Form used for?

This form registers a new employee in the General Provident Fund scheme and assigns the unique GP Fund account number against which all later monthly deductions, advances, interest credits, and the retirement settlement are recorded. Without an allotted number, your contributions cannot be tracked and no advance or final payment can be sanctioned.

Who needs to fill out this form?

Every newly appointed regular Punjab government employee who does not yet hold a GP Fund number must submit this form through their Drawing and Disbursing Officer (DDO). It is also required in a few specific situations:

  • Newly inducted regular government employees
  • Employees transferred in without an existing GP Fund record
  • Staff whose earlier allotment was never completed or was lost
  • Employees rejoining service after a break

  • Date of birth and date of appointment
  • Designation, office, and personal / pay number

  • Office order or joining report
  • Nominee's CNIC copy

Important tips to avoid rejection

Most allotment forms are returned for avoidable reasons. Keep these points in mind before submission:

  • Ensure your CNIC number is correct and matches the attached copy.
  • Do not leave any field blank; mark 'N/A' where a field does not apply.
  • Nominee details must be complete; an incomplete nomination causes problems later.
  • All official signatures and office stamps must be present.
