What documents must be attached?

- Original bills and cash memos from the hospital or pharmacy
- Discharge summary / medical prescription
- Hospital's stamped and signed medical certificate
- Copy of the patient's CNIC and relationship with the employee
- Referral letter, if treatment was taken on reference
How do I submit the medical claim?
Follow these steps for a smooth reimbursement:
- Download the docket in Word and fill in the employee, patient, and treatment details.
- Attach all original bills and supporting documents.
- Obtain the DDO / head of office signature and stamp.
- Submit to the concerned medical reimbursement authority for sanction.
